All Requests Should Be Made With Our Order Form
This is a "pdf" version of the order form, and is the preferred one to use. Print it and fill it out. Then either scan it and attach it to an email, or mail it through the post office.
This is a "text" version of the order form. Save it and edit it electronically with any text editor, then attach it to an email. Only use this version if you want to email the form but cannot scan the pdf version. Also use this version if you don't have pdf viewing software. Use the pdf version if at all possible.
